Intracytoplasmic sperm injection, or ICSI, is a procedure that involves injecting a single live sperm directly into the center of a human egg. This fertilized egg is then transplanted into the female's body.
ICSI is particularly useful for couples with severe male factor infertility. It is also helpful for couples who have had no success with previous IVF attempt(s). High success rate in ICSI has made it one of the most popular procedures.
In the case of male factor infertility with an abnormal semen analysis, ICSI is regarded as absolutely important. Patients are opting for ICSI for a variety of reasons other than male factor infertility, such as:
Men with serious infertility concerns, such as a low sperm count, are ideal candidates for ICSI. Men with sperm motility abnormalities, sperm development problems, or an obstruction that prevents sperm from being ejaculated, such as a vasectomy, are also prospects.
ICSI may be beneficial to couples who have not had success with previous in vitro fertilization attempts. Couples who want to test for specific genetic abnormalities are also candidates for ICSI treatment.
The first step involves ovarian stimulation. During this step, the doctor will make an appointment to do an ultrasound and blood test to check the quantity and size of the eggs, and whether they are mature for further procedures. After verifying that the eggs are large enough, an hCG injection will be given 36 hours before the egg retrieval surgery to promote complete maturation.
The retrieval of eggs and sperm is the next step. During this process, the female will be made to sleep. The doctor will use a vaginal ultrasound to find the location and determine the number of eggs. The eggs are aspirated out of the vaginal canal using a needle. Sperm is, on the other hand, collected in a private room.
Following the egg and sperm retrieval, the embryologist will choose the healthiest egg and sperm to fertilize by injecting only one best sperm directly into the egg with a microneedle. The embryo will either be put back into the uterine cavity or frozen until a future need arises. After that, an abdomen ultrasound is performed to determine the best site for embryo implantation. The embryo is then transferred to the uterus through the cervix (does not require anesthesia).
ICSI cycles might take anywhere from four to six weeks to complete. The egg and sperm retrieval procedures will take around a half-day for you and your spouse at the clinic. For the embryo transfer operation, you will often be asked to return in two to five days.
Many couples with difficult male factor infertility issues have been able to conceive thanks to the ICSI technique. Fertilization rates of 60 to 80 percent of all eggs injected are currently being accomplished, which is identical to fertilization with normal sperm, and pregnancy rates are comparable to those reported with IVF in couples with no male factor infertility.

The way the sperm fertilizes the egg is the main difference between IVF and ICSI. The egg and sperm (which are abundant) are left in a laboratory dish to fertilize on their own in IVF. The selected sperm is directly injected into the egg during ICSI.
ICSI has a high success rate in assisting sperm and egg fertilization. However, many other factors, including the woman's age and whether she has any reproductive issues, still play a role in a healthy pregnancy, just as they do with IVF. ICSI has success rates that are pretty similar to IVF.

ICSI procedure- needle aspiration and injection may cause some discomfort to the patient. The anesthesiologist will use anesthetic and sedative drugs to make the egg retrieval procedure as painless as possible.
After 12-14 days after the embryo transfer, the doctor will schedule a blood test to assess the hCG hormone level. The test will reveal whether the pregnancy is successful or not.

There are some risks to consider. First, a small percentage of eggs - usually less than 5% - may be destroyed as a result of the needle insertion during the ICSI method. Second, the total risk of having a baby with an X or Y chromosome defect is 0.8 percent, or eight per 1000, which is four times the rate found with spontaneous conception.
