What is Hip Replacement Surgery?

Everyday tasks such as walking or getting in and out of a chair may be uncomfortable and difficult if your hip has been injured by arthritis, a fracture, or other disorders. Hip replacement surgery is one of medicine's most successful procedures. Hip replacement has become much more effective since the early 1960s, thanks to advancements in joint replacement surgical techniques and technologies. Each year, more than 450,000 total hip replacements are performed in the United States, according to the Agency for Healthcare Research and Quality.

Hip Joint

The hip is one of the biggest joints in the body. It is a ball-and-socket joint. The acetabulum, which is a portion of the pelvis bone, forms the socket. The femoral head, or top end of the femur, is the ball (thighbone).

Articular cartilage covers the bone surfaces of the ball and socket, a smooth tissue that cushions the ends of the bones and allows them to move freely.

The hip joint is surrounded by a thin tissue called the synovial membrane. This membrane produces a little amount of fluid that lubricates the cartilage and reduces friction during hip movement in a healthy hip.

Ligaments (the hip capsule) attach the ball to the socket and offer support to the joint.

Common Causes of Hip Pain
  • Arthritis is the most common cause of chronic hip pain and impairment. The most frequent types of arthritis include osteoarthritis, rheumatoid arthritis, and traumatic arthritis.
  • Osteoarthritis. This is a "wear and tear" type of arthritis that develops with age. It mainly affects adults over the age of 50, and it's more common in persons who have a family history of arthritis. The cartilage that cushions the hip bones wears away. Hip pain and stiffness result from the bones rubbing against each other. Osteoarthritis can also be triggered or hastened by minor flaws in how the hip grew as a youngster.
  • Rheumatoid arthritis is a type of arthritis that affects the joints. The synovial membrane becomes inflamed and thickened in this autoimmune illness. Chronic inflammation can cause cartilage degradation, resulting in pain and stiffness. Rheumatoid arthritis is the most frequent kind of an inflammatory arthritis group of diseases.
  • Post-traumatic arthritis Arthritis caused by a traumatic event. This can happen as a result of a major hip injury or fracture. Over time, the cartilage may be destroyed, resulting in hip discomfort and stiffness.

Hip Replacement Surgery in India

The diseased bone and cartilage are removed and replaced with prosthetic components in a hip replacement (also known as total hip arthroplasty or total hip replacement).

The injured femoral head is removed, and a metal stem is inserted into the hollow middle of the femur to replace it. In the bone, the femoral stem can be cemented or "pressed fit."

On the upper half of the stem, a metal or ceramic ball is put. This ball replaces the femoral head that was lost due to injury. The acetabulum's injured cartilage surface is removed and replaced with a metal socket. The socket is occasionally held in place with screws or cement.Between the replacement ball and the socket, a plastic, ceramic, or metal spacer is inserted allowing for a smooth gliding surface.

  1. Total and Partial Hip Replacement (Hip Ball Replacement) – Which parts of a total or partial hip replacement must be replaced?Both the ball and the socket are replaced during total hip replacement (total hip arthroplasty).Hip Ball Replacement SurgeryHip ball replacement is also known as partial hip replacement. Traditional or Minimally Invasive Hip Replacement- How long will the incision be?Traditional Hip ReplacementA single, big incision is used in a traditional hip replacement to allow the surgeon access to the hip, which is commonly done from the side (lateral approach) or from the rear (back approach) (posterior approach).Because the surgeon must cut through or detach several muscles and tendons to access the joint, a typical hip replacement can take a long time to recover from. The muscles and tendons are also repaired when the implant is placed.Minimally Invasive Hip ReplacementThe goal of minimally invasive hip replacement is to keep the impact of surgery on healthy tissues like muscles and blood arteries to a minimum.While anterior hip replacement has been touted as a minimally invasive procedure, orthopedic surgeons today use minimally invasive procedures to access the hip with all surgical approaches. Your surgeon will talk to you about which strategy will give you the best results.The surgeon enters the hip joint by one or two small incisions by pulling the muscles aside during minimally invasive surgery. This strategy may provide a number of advantages, including:
    • Muscle injury is less likely.
    • There is less discomfort.
    • Recovery will be quicker and easier.
    • There will be less limping.
    • A shorter stay in the hospital
    • Hip dislocation is less likely.

    It is important to note that not all patients are candidates for minimally invasive hip replacement. Your age, weight, degree of fitness, and other considerations will assist the surgeon in determining whether or not you are a good candidate for minimally invasive hip replacement.

There are two pieces to a hip replacement implant, commonly known as a hip prosthesis:

Ceramic Ball- For stability, a ceramic ball is linked to a metal stem that is placed into the thigh bone (femur).

Metal Cup- The acetabulum is a metal cup with an inner plastic covering that is attached to the socket portion of the hip joint (acetabulum) to allow the prosthetic joint to move smoothly.

The ball used to be composed of metal, but ceramic is now the industry standard.

Uncemented– The uncemented prosthesis is attached to a porous surface that permits bone to develop and hold it in place over time.
Cemented– Bone cement is used to secure the cemented prosthesis.

Both uncemented and cemented approaches to implant security can be effective. The cement used in hip replacement surgery has improved over time, as have strategies for encouraging natural bone regrowth.

The surgeon may use a combination of procedures for some hip replacements. He or she may decide to utilize cement on the femoral stem while leaving the socket component that inserts into the hip bone uncemented.

When is Hip Replacement Surgery required?

You, your family, your primary care physician, and your orthopedic surgeon should all work together to make the choice to have hip replacement surgery. Your doctor may prescribe hip replacement surgery for a variety of reasons few of which include:

  • Hip pain that prevents you from doing things like walking or bending
  • Hip pain that persists while resting, whether during the day or at night
  • Hip stiffness that makes it difficult to move or lift the leg
  • Anti-inflammatory medicines, physical therapy, or walking aids have not provided adequate pain relief.

There are no age or weight restrictions in being a candidate for hip replacement surgery. Surgery is recommended based on a patient's discomfort and handicap, not their age. The majority of patients who have a total hip replacement are between the ages of 50 and 80, although orthopedic surgeons assess each patient individually. Hip replacements have been successfully performed on patients of all ages, from teenagers with juvenile arthritis to the elderly with degenerative arthritis.

How is Hip Replacement Surgery done?

The surgery is usually performed under general anesthesia and takes two hours.

Step 1

An incision is made on the lateral side of the hip. The upper round part of the thigh bone (head of the femur) is removed and replaced with an implant.

Step 2

It is then stabilized into place by cementing or other methods. The part of the hip joint receiving the ball component called a socket or acetabulum is smoothened and prepared, and the implant is then placed to position and stabilized.

Step 3

The new ball and socket of the joint are then joined.

What is the Recovery after Hip Replacement Surgery like?

The healing process for a hip replacement begins straight away. After surgery, you will be encouraged to get up and move around as soon as possible. You'll be taken to a recovery area for a few hours after surgery when your anesthetic wears off. Your blood pressure, pulse, awareness, pain or comfort level, and medication needs will all be monitored by medical personnel.

The length of your stay after surgery is determined by your specific needs. The stay at the hospital ranges from 3-5 days. To help keep fluid out of your lungs, you'll be asked to take deep breaths, cough, or blow into a device.

Physical therapy will start the next day and shall be required for several weeks after surgery. This will help you rebuild muscular strength and range of motion. When you are ready to walk with or without help, the surgeon, physical therapist, or occupational therapist can advise you on how to manage your discomfort. Your rehabilitation needs will be discussed, as well as what to expect in the days and weeks ahead and how to make the most of your recuperation. Loosening. Although this is a rare issue with newer implants, your replacement joint may not be firmly attached to your bone or may loosen over time, producing hip pain. Surgery may be required to correct the condition.

Damage to the nerves. Nerves in the location where the implant is put can be harmed on rare occasions. Numbness, weakness, and discomfort are all symptoms of nerve injury.

A careful surgical technique, the use of a pneumatic tourniquet, hypotensive anesthesia, proper patient placement, and maintaining the patient's body temperature throughout surgery all help to reduce blood loss.

Yes, both hips can be replaced during a single operation, and the procedure for replacing both hips during a single surgery is called bilateral hip replacement. If you have problems with both hips and are in good health, your doctor may recommend a double hip replacement, provided you can handle a lengthier surgery and a more difficult recovery.
